It is so easy to transpose or misread information. We are all in a hurry, often required to enter data or responses within fixed time limits so that mistakes are made.
I once worked for a support company that assumed all issues could and should be resolved in 15 minutes. As you talked to the customer trying to sort out the problem, a red light appeared and grew in intensity as you approached the 15-minute limit and hadn't hung up. Of course, your failures were recorded and brought up during reviews.
